Job Description

We're looking for a talented freelancer to design a comprehensive vendor form that meets the unique needs of our U.S.-based medical device business. The ideal candidate will have expertise in LaTeX, Adobe Acrobat, or similar tools for form design, as well as experience creating business forms or templates. Attention to detail and a deep understanding of U.S. business and tax requirements are essential for this role.

Key Responsibilities

As a freelance form designer for arenaflex, you'll be responsible for creating a professional vendor form that includes the following sections:

• Customer Information: A section for collecting essential customer details, including:

+ Legal name + DBA (Doing Business As) name + Billing address

• Contact Details: A section for capturing key contact information, including:

+ 2 Accounts Receivable contacts + 3 key contacts + Optional Head Service Person

• Tax and Compliance Info: A section for collecting relevant tax and compliance information, including:

+ EIN (Employer Identification Number) + W-9 + FDA registration #

• Purchasing, Shipping, and Payment Details: A section for collecting information related to purchasing, shipping, and payment, including:

+ Purchase order details + Shipping information + Payment terms

• Required Documentation: A section for collecting required documentation, including:

+ W-9 + Purchase order + Signed form

Deliverables

As a freelance form designer for arenaflex, you'll be expected to deliver the following:

• A fully functional PDF form template
• Source files (e.g., LaTeX code or design file) for future edits
• Brief instructions on how to use/edit the form
