Job Summary

We are seeking an Experienced Licensed Social Worker to join our remote medical case management and advocacy team. As a member of our team, you will work collaboratively with a registered nurse case manager to provide comprehensive support to members, empowering them to make informed decisions about their health and well-being. Your expertise and empathy will be invaluable in helping members navigate complex healthcare systems, access community resources, and overcome financial, transportation, safety, and housing challenges.

Responsibilities

* Partner with a registered nurse case manager to provide comprehensive support to members, empowering them to make informed decisions about their health and well-being

• Conduct thorough assessments of members' needs, identifying areas where they require support and developing personalized plans to address these needs
• Research and connect members with community resources, including financial assistance programs, transportation services, and housing options
• Provide education and support to members on healthy lifestyle choices, disease management, and medication adherence
• Collaborate with other clinical staff to ensure seamless care coordination and effective communication with members
• Maintain accurate and up-to-date records of member interactions, including assessment results, plans, and outcomes
• Participate in ongoing training and professional development to stay current on best practices in medical case management and advocacy

Requirements

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence

• 2+ years of experience in medical social work or case management
• Master's degree in Social Work or related field
• Excellent communication and interpersonal skills, with the ability to build rapport with members and colleagues
• Strong assessment and problem-solving skills, with the ability to think critically and creatively
• Ability to work independently and as part of a team, with a high level of flexibility and adaptability
• Proficiency in electronic medical records and other relevant software systems
