Job Summary

As a Case Management Specialist, you'll be responsible for providing high-quality, patient-centered care to individuals enrolled in our DSNP program. You'll work closely with healthcare providers, community organizations, and other stakeholders to develop and implement comprehensive care plans that address the unique needs of each individual. Your expertise, compassion, and dedication will make a tangible difference in the lives of our patients and their families.

Key Responsibilities

* Conduct thorough assessments of individuals' needs and develop personalized care plans that address their unique health and social challenges

  • Identify high-risk factors and service needs that may impact patient outcomes and develop strategies to mitigate these risks
  • Collaborate with healthcare providers, case managers, and other stakeholders to ensure seamless care coordination and optimal patient outcomes
  • Develop and implement care plans that address the full continuum of an individual's health and social needs
  • Provide education, support, and resources to empower individuals to make informed decisions about their healthcare and well-being
  • Facilitate communication between patients, healthcare providers, and other stakeholders to ensure timely and effective care coordination
  • Monitor and evaluate patient progress, making adjustments to care plans as needed to ensure optimal outcomes
  • Document all interactions, assessments, and care plans in accordance with regulatory requirements and company policies

Essential Qualifications

* 3+ years of experience in a health-related field, with a strong understanding of healthcare systems and policies

  • Bachelor's degree or equivalent experience in a related field (e.g., healthcare administration, social work, nursing)
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, healthcare providers, and other stakeholders
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously

Preferred Qualifications

* Certification in case management (CRC, CDMS, CRRN, COHN, or CCM)

  • Experience working with geriatric special needs, social health, and disability populations
  • Familiarity with evaluation, screenings, and care planning processes
  • Bilingual (English/Spanish; English/Creole) or multilingual skills
  • Experience working with federal health insurance and Medicaid programs
