Remote Patient Care Coordinator

Responsibilities:

• Schedule nurses and Home Health Aides (HHAs) to meet patient needs.
• Utilize intermediate to advanced proficiency with Microsoft Office Suite (Word, Outlook, Excel).
• Participate in a weekend on-call rotation.
• Communicate shift confirmations to the care team.
• Manage trackers and spreadsheets to ensure efficient scheduling and resource allocation.
• Monitor nurse and HHA visits, gathering patient satisfaction feedback through calls.
• Employ various technology platforms for tracking, time management, and report generation.
• Collaborate with different departments to address diverse patient needs.
