Case Manager - Behavioral Health

Responsibilities:

• Conduct comprehensive assessments of members' needs and develop care plans to address their unique requirements
• Collaborate with healthcare providers to ensure seamless care coordination and communication
• Provide education and support to empower members to make informed decisions about their healthcare
• Monitor and evaluate the effectiveness of care plans and make adjustments as needed
• Utilize care management tools and resources to support members' needs and goals

Requirements:

• Minimum 2 years of experience in behavioral health or social services
• Willingness to travel up to 50-75% of the time to meet with members in person
• Reliable transportation and ability to reimburse mileage according to company policy
• Active IL BH clinical license (LCSW, LCPC, LMFT)

Preferred Qualifications:

• Case management and discharge planning experience
• Managed care experience
• Microsoft Office experience
