Centers Health Care is seeking compassionate and experienced Licensed Practical Nurses (LPNs) to join our team on a part-time basis at our Skilled Nursing Facility in Brightwaters, NY. As an LPN at Centers Health Care, you will play a vital role in delivering high-quality patient care and supporting our team of healthcare professionals.
About the Role:
- Collect and record patient information, including medical records and insurance details
- Monitor and record patient vital signs, including temperature and other health metrics
- Administer medications and injections as prescribed by physicians
- Provide wound care and dressing changes as needed
- Supervise and support Certified Nursing Assistants (CNAs) as required
- Assist patients with daily living activities, including dressing and personal hygiene
- Monitor patient food and liquid intake and output
Requirements:
- Valid state LPN license
- Strong teamwork and communication skills
- In good standing with the State Registry
