Job Overview Join our dedicated team as a Licensed Social Worker, where your compassion and expertise will directly impact individuals and families affected by ALS. This remote position allows you to connect meaningfully with patients and caregivers, providing vital support during their most challenging times... Key Responsibilities • Conduct home visits and perform comprehensive social work assessments, counseling, case management, and crisis intervention. • Co-facilitate support groups, offering a safe space for patients and their caregivers. • Organize and coordinate monthly educational programs to empower and inform stakeholders. • Engage in chapter-wide events to foster community connection and outreach. Required Skills • Active New York State LSW license. • Minimum of three years of relevant experience in social work, with an emphasis on end-of-life care. • Exceptional communication and interpersonal skills. • Ability to work autonomously as well as collaboratively within a team environment. • Bilingual capabilities are advantageous. • Valid driver’s license and access to a vehicle. Qualifications • Licensed Social Worker (LSW) with a valid New York State license. • At least three years of direct experience in a similar capacity. • Demonstrated knowledge and experience in end-of-life care settings.
