Position: Community Health Worker... Location: Remote/WFH - Must be Resident of North Carolina Duration: 3 Month Contract to start, potential for extension and/or conversion to FTE Rate: $22.00/Hr Start Date: 11/04/2024 Important: Must be local resident of North Carolina Primary Responsibilities: As a Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate services are obtained via the Healthy Opportunities Pilot Program in addition to care coordination and member education. Primary Responsibilities: • Perform social needs assessment for members • Utilize the NCCARE360 platform for documentation and referral purposes • Clearly and effectively communicate with members and Human Service Organizations to arrange services • Linking members to applicable community resources • Telephonic outreach to targeted members • Proactively engage the member to manage their care • Provide member education • Support transitions of care • Create a positive and engaging experience for the member • Help to keep members compliant with their care plans • Partner with and support care team by completing delegated tasks • Knowledge and continued learning of community cultures and values • Conduct individual-level screenings to capture member's current health care needs (e.g. Transitions of Care, Health Risk assessments, Pilot Eligibility Service Assessment (PESA) • Engage member to participate in the screening process and collaboratively develop plan of care based on their individual needs, preferences, and objectives Required Qualifications: • High School Diploma / GED (or higher) • Must be a North Carolina resident • Must have a designated workspace inside the home with the ability to access high-speed internet • Must be proficient in Outlook and Microsoft Office • Must possess excellent organizational skills • Must be proficient in clear, customer service focused communication Would be a Plus to Have: • Bachelor’s Degree (or higher) in Social Work and / or Health Care Administration • Licensed Practical Nurse (LPN), Certified Nursing Assistant / Home Health Aide, Medical Assistant • CHW Accreditation • Experience working in Managed Care and/or Social Needs assessment • Knowledge of Medicaid population Hours: Remote Work/telecommute and must live in North Carolina (EST) Hours: 8 AM – 5 PM EST, Monday-Friday, with a 1 hour lunch Equity Staffing Group is one of the fastest growing Native American owned staffing companies in the United States.
