You will assess patient needs, provide education and support, and collaborate with healthcare providers to develop care plans. The ideal candidate will have a strong background in nursing, excellent communication skills, and a passion for helping patients manage their health. Requirements: 1. Active RN license 2. Minimum of 2 years of clinical experience 3. Strong computer skills 4. Ability to work independently and within a team 5. Excellent communication and interpersonal skills 6. Knowledge of chronic disease management 7. Ability to work in a fast-paced environment Responsibilities: 1. Conduct telephonic assessments of patient needs 2. Develop and implement care plans in collaboration with healthcare providers 3. Provide education and support to patients on managing their chronic conditions 4. Document patient interactions in electronic health record system 5. Coordinate care with other healthcare providers as needed 6. Follow up with patients to monitor progress and adjust care plans as necessary 7.
