Job Description: We are seeking experienced Discharge Coordinators from hospitals in Bergen County to provide consultation servicesto our intake process at Live Easy Home Care. The aim is to leverage your expertise in hospital discharge planning and senior home care referrals to enhance our operations and optimize our services for clients transitioning from hospital to home care. Responsibilities: • Provide consultation around the intake process for senior patients referred to Live Easy Home Care. • Share insights into discharge planning, hospital protocols, and effective coordination with home care agencies. • Offer recommendations for improving our intake procedures to better align with discharge planning standards. • Contribute to strategy development for better integration of seniors into our care system. • Offer advice on improving communication between discharge planners and home care providers to enhance patient outcomes. Requirements: • Experience as a Discharge Coordinator or in a similar capacity in a hospital setting. • Strong understanding of the discharge process and how to connect patients to post-hospitalization services, especially home care. • Knowledge of senior care services and an understanding of the needs of the elderly during transition from hospital to home care. • Excellent communication skills and ability to work collaboratively with our home care team. • Ability to provide actionable and practical recommendations that align with home care industry standards.
