Licensed Practical Nurse – Chronic Care Management (Remote)
Job Summary: SAS Health Tech is looking for a Licensed Practical Nurse (LPN) to join our growing remote Chronic Care Management team. In this role, you will play a key part in supporting patient care through remote telehealth services. Key Responsibilities:
- Monitor and review patient symptoms and medications.
- Address patient inquiries about their health during scheduled telehealth sessions.
- Collaborate with facility CNAs to arrange remote telehealth visits.
- Create detailed 20-minute patient entries and update records in our EHR system.
- Develop and implement individualized care plans based on established protocols.
- Stay informed about patient care policies, client rights, and evolving healthcare guidelines.
- Recognize signs of improvement or decline in various medical conditions.

Requirements:
- A valid Florida LPN license or licensure in a compact state.

Position Details:
- Type: Full-time, independent contractor role
- Compensation: $23.00 – $25.00 per hour, with opportunities for bonus incentives (e.g., completion and monthly bonuses).
- Preferred Medical Specialty Experience: Cardiology, Geriatrics, Home Health, Internal Medicine, or Primary Care.
