In this position, you will be responsible for providing comprehensive case management to a diverse group of patients, ensuring the delivery of quality care, and supporting them through the healthcare journey. Key Responsibilities: Assess, plan, implement, and evaluate care plans for patients, ensuring all care needs are met. Coordinate with healthcare providers and other team members to ensure the best care for patients. Develop and maintain relationships with patients, assisting them in navigating healthcare services and resources. Monitor and manage the health progress of patients with chronic or complex conditions. Provide education to patients and their families on managing healthcare conditions. Collaborate with interdisciplinary teams to promote patient-centered care. Maintain accurate documentation of patient care, progress, and outcomes. Ensure compliance with company policies, industry regulations, and quality standards. Requirements: Active Registered Nurse (RN) license in good standing. Minimum of 3 years of clinical nursing experience. Strong knowledge of case management processes and healthcare coordination. Ability to work independently and manage multiple priorities in a remote environment. Excellent communication, interpersonal, and problem-solving skills. Familiarity with electronic medical records (EMR) and other healthcare technology. Experience in home health, geriatrics, or working with chronic conditions is preferred.
