The social worker will conduct comprehensive assessments to establish personalized Plans of Care, collaborating with healthcare providers to ensure patients' needs are met. Key Responsibilities: • Assess patients for appropriateness to the Critical Assess setting, ensuring all criteria are met for hospital and/or swing bed program admission • Conduct comprehensive assessments of patients' needs, strengths, goals, life history, and preferences • Develop individualized Plans of Care in collaboration with the physician and nursing team to ensure all needs are addressed • Work with patients and their families to provide support, guidance, and education regarding care plans and available resources • Refer patients to appropriate external services and assist in navigating community resources • Document assessments, interviews, and Plans of Care in patient records • Ensure that all patients receive the necessary services and resources during their stay Qualifications: • Strong knowledge of medical social work practices, patient care protocols, and available community resources • Excellent interpersonal and communication skills, with the ability to work effectively within a multidisciplinary team • Compassionate and patient-focused with the ability to provide emotional support to patients and families Ballinger Memorial Hospital District is committed to providing high-quality patient care and improving health outcomes in our community.
