Job DescriptionJob DescriptionCompass Healthcare Consulting and Placement is currently seeking bilingual Spanish or Cantonese/Mandarin speaking Registered Nurses for Hybrid then Remote RN Care Manager opportunity working for Managed Care Plan. Position is hybrid 2 days in Manhattan office, 3 days from home and then fully remote after 4-6 months, office located in Manhattan, NY. Qualified candidates must have NYS-RN License and bilingual English/Spanish, English/Mandarin or English/Cantonese language abilities along with 1-3 years of Home Health RN and/or MLTC Care Management or UAS Assessment RN experience.The RN Care Manager provides clinical support and oversight of care that ensure members are receiving safe and effective care in the communityQualifications:• RN (BSN preferred). Must have current, valid, and unencumbered license, certificate or registration or credentialing as required.• Experience as a NY licensed RN working in a home care environment or in an acute, sub-acute, or long-term care (LTC) setting or managed long-term care (MLTCP) required, Will train bilingual RNs with Home Health experience!• Must maintain all state and federal compliance requirements for licensure and employment.• Bi-lingual Spanish, Chinese, Mandarin, Cantonese or Korean. UAS certified, and knowledge of care management or Home Care preferred.Essential Job Responsibilities:• Responsible for service authorizations, creates care plans, and coordinates services throughout the continuum of care for high-risk populations.• Assesses and coordinates Hospitalization / Skilled Services CHHA /Nursing Home Communication and Planning.• Coordination of Benefits for Medicare, Medicaid, Other insurance.• Coordinates transitional care management (acute to home, home to skilled nursing facility SNF/custodial, home to hospital and acute to SNF/custodial.• Completes Monthly Call Assessments and follows up on calls triggered by changes in meds, diagnosis or events such as ER visits, falls, etc.• Coordinates post-discharge services as needed.Benefits: Generous PTO • 403(b) • Dental insurance • Employee assistance program • Health insurance • Life insurance • Paid time off • Vision insuranceSalary $95,000.00 - $110,000.00 per year, based on previous experience, $15,000 Bonus & Excellent Benefits including generous PTO and Employer Contribution to a Retirement Plan!Qualified Candidates Please Apply Now for Immediate ConsiderationCompany DescriptionCompass Healthcare Consulting & Placement is a Permanent Placement Recruitment Company.
