Your Role Provide monthly calls and support to patients enrolled in the CCM program. Utilize your nursing skills to educate patients and their families. Monitor patients progress and work with the physicians to ensure quality patient care. Qualifications Active LPN license (compact) Experience in geriatrics or primary care Ability to sit and talk on the phone for long periods of time System: We require you to have a Windows i5 (or newer) processor, and to have Windows Pro (not Home). This can be a laptop or desktop. Must live and work full time in the US - you can work from anywhere within the US but not outside. The Pay The LPN pay ranges from $21.85 - $23.77 per hour (40hrs/week) The pay is a base pay of $17.50/hr plus a bonus of $2.00 per call. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter. Apply Now and start making a meaningful impact with us!
