About the position The Urgentivist Extender RN Urgent Home Visits role at Landmark, part of the Optum family, involves providing medical care and management to patients in their homes who require urgent visits. This position is integral to the Homebased Medical Care program, focusing on improving patient health and preventing hospitalizations through timely interventions. The role offers flexibility with a hybrid work model, allowing for both home visits and administrative tasks from a home office, while collaborating with interdisciplinary teams to enhance patient care. Responsibilities • Practices in accordance with state laws and regulations governing registered nurses. , • Responds to urgent patient needs to assess illness acuity and prevent hospitalization through home interventions. , • Communicates with the longitudinal provider and/or pod leader during patient visits to discuss assessments and implement orders. , • Performs basic nursing procedures such as IV placement, blood draws, injections, and wound care as directed by supervising providers. , • Completes required documentation promptly to ensure care team access and co-signature. , • Demonstrates competency in managing chronic disease conditions and their exacerbations. , • Participates in discussions regarding urgent call matters that may require follow-up or care plan alterations. , • Completes follow-up and post-hospital assessments according to standard operating procedures. , • Implements diagnostics and interventions as directed by providers to avoid unnecessary admissions. , • Collaborates with medical leadership to develop quality improvement programs and provide feedback on equipment and training needs. , • Educates patients and caregivers on disease management and care processes. , • Stays current on medical and nursing research through continuing education and professional meetings. Requirements • Current/unrestricted RN license in the state of Florida. , • 2+ years clinical experience as a Registered Nurse. , • Proven ability to conduct home visits for urgent care, even in inclement weather. , • Demonstrated proficiency with Microsoft Office applications. , • Ability to work Monday - Friday from 10:00am to 7:00pm ET. , • Distraction-free home office space. , • Access to secure high-speed internet. , • Reliable transportation compliant with Optum Motor Vehicle Safety policy. , • Willingness to travel up to a 60-minute radius from Bradenton, FL. Nice-to-haves • Experience in home health, geriatrics, ICU/ED, urgent care, and palliative care. , • Video conferencing experience. , • Knowledge of inpatient, skilled nursing, community resources, and cost-effective patient care delivery systems. , • Familiarity with clinical standards of care, CMS guidelines, and Medicare contracts. , • Competency in managing chronic disease conditions and their exacerbations.
