Experienced Home Health Registered Nurse - PRN - Flexible Remote Work Arrangement

Key Responsibilities
- Coordinates and oversees all aspects of patients' home care, ensuring that their needs are met and that they receive high-quality care.
- Collaborates with patient care teams to develop and implement care plans that are tailored to each patient's unique needs.
- Educates families on patient condition, needs, and care, empowering them to take an active role in their loved one's healthcare.
- Performs head-to-toe physical assessments, checks vital signs, and manages medications to ensure that patients receive comprehensive care.
- Gives injections, IV therapy, or tube feedings as needed, and administers and maintains catheters to support patients' medical needs.
- Completes documentation timely and accurately, ensuring that patient records are up-to-date and reflect the care provided.
- Supervises LPNs and HHAs, providing guidance and support to ensure that patients receive seamless care.
- Regularly communicates patient progress to the clinical manager, physician, and care team, ensuring that all stakeholders are informed and aligned.
- Other duties as assigned, demonstrating flexibility and a commitment to delivering exceptional patient care.
Qualifications and Requirements To be considered for this role, you must meet the following qualifications:
- One year of clinical experience as a registered nurse (RN). If you have less than 1 year of clinical experience, you must be approved by our VP of Clinical Services.
- Current RN license, specific to the state(s) you are assigned to work.
- Minimum six months of home health experience, demonstrating your ability to provide care in a home setting.
- Current CPR certification, ensuring that you are equipped to respond to emergencies.
- Valid driver's license, reliable transportation, and liability insurance, enabling you to travel to patient homes.
- Ability to assess patient status and identify requirements relative to age-specific needs, demonstrating your understanding of patient care.
- Familiarity with general use and functions of the computer, including user names and password concepts, internet, e-mail, and navigation of computer desktop.
If you have less than 6 months of clinical experience as an RN, you will be required to participate in our RN Intern program, which will provide you with the support and training you need to succeed in this role.
