Experienced Licensed Social Worker – Remote Case Management and Advocacy

Job Summary

We're seeking an experienced Licensed Social Worker to join our team in a remote case management and advocacy role. As a key member of our team, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a diverse population across the entire lifecycle, utilizing telephonic and electronic communication to empower members to make positive lifestyle and medical choices.

Responsibilities

- Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs - Research available options and resources for members with financial, transportation, safety, and housing needs - Empower members to make positive lifestyle and medical choices through telephonic and electronic communication - Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health - Develop and implement individualized plans to address members' needs and promote positive outcomes - Maintain accurate and up-to-date records of member interactions and progress - Participate in ongoing education and training to stay current with best practices and industry developments

Requirements

- Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence - 2+ years of experience in medical social work or case management - Masters Degree in Social Work - Strong communication and interpersonal skills, with the ability to build rapport with diverse populations - Ability to work independently and as part of a team, with a focus on collaboration and problem-solving - Strong analytical and problem-solving skills, with the ability to think critically and creatively - Proficiency in electronic communication and documentation systems - Ability to maintain confidentiality and handle sensitive information with discretion
