We\'re also committed to building a diverse, inclusive culture where all employees have the potential to grow professionally and personally.What We\'re Looking ForThe Innovations Waiver Care Manager is responsible for providing complex care coordination and monitoring to members having a primary Intellectual or Developmental Disability, which may include a secondary mental health or substance abuse, and/or complex medical/physical health need. This position requires a dynamic, proactive approach to assessment, monitoring and complex coordination of care, to ensure quality support and consistent adherence to waiver requirements. The Innovations Waiver Care Manager assists with integrated care management through multi-disciplinary collaboration.On a Typical Day, You MightUtilize person-centered planning methods and strategies to gather information, to get to know the members supported and to adequately address specific needs of the individual.Provide education and support to members on learning about and exercising rights and responsibilities.Complete discovery activities (information gathering and assessment) as a component of the planning process.Ensure that assessments and plans are updated as needed and whenever the member\'s life circumstances change.Employee BenefitsTrillium knows that work/life balance is important. QualificationsEducation/Experience:Must meet QP status.Fully licensed by the NC governing board regulating H man Servicesprofessions; OR
  • Fully licensed RN** who is licensed to practice in the state of NC by the NC
Board of Nursing who also has four (4) years of full-time experience with theMH/DD/SA population served. ORgraduate of a college or university with a Master\'s degree in a humanservice field and has one year of full-time, pre or post-graduate degreeaccumulated MH/DD/SA experience with the MH/DD/SA population, or asubstance abuse professional who has one year of full-time, pre orpostgraduate degree accumulated supervised experience in alcoholismand drug abuse counseling; ORA graduate of a college or university with a bachelor\'s degree in a humanservice field and has two years of full-time, pre or post-bachelor\'s degreeaccumulated MH/DD/SA experience with the MH/DD/SA population, or asubstance abuse professional who has two years of full-time, pre or postbachelor\'s degree accumulated supervised experience in alcoholism anddrug abuse counseling; OR
