Position Summary We are seeking a motivated and detail-oriented Medical Assistant to join our Chronic Care Management team. In this role, you will play a critical role in patient care by guiding patients through health assessments, reviewing medications and diagnoses, troubleshooting medical devices, and escalating clinical needs to nursing staff when appropriate.

Key Responsibilities

• Support CCM nursing staff by completing monthly patient assessments and conducting Chronic Care Planning Assessment (CCPA) calls, ensuring a positive, informative, and patient-centered experience.
• Complete delegated clerical and administrative activities on behalf of nursing staff to support patient engagement, improve care coordination, and drive quality outcomes and overall patient experience.
• Document patient interactions, assessments, and intake in the EHR
• Maintain compliance with company policies and applicable regulations
• Coordinate communication between patients, caregivers, nursing staff, and providers to ensure continuity of care and timely follow-up on patient needs.
• Perform other duties, as assigned.

Qualifications

Required:

• Active MA license required.
• Strong communication and organizational skills

Preferred:

• Passionate about patient care, possessing the ability to relate with empathy and compassion. A passion for patient teaching is a must.

Work From Home Requirements

Schedule

• Monday - Friday 8:00 am - 5:00 pm EST
• Monday - Friday 9:00 am - 6:00 pm EST
• Monday - Friday 10:00 am - 7:00 pm EST
• Monday - Friday 11:00 am - 8:00 pm EST

Training Requirements All new hires must complete a comprehensive training program:

• Duration: Five weeks

Schedule: Monday through Friday, 9:00 AM – 6:00 PM Eastern Time

• Attendance is mandatory to ensure readiness prior to independently supporting patients.

Compensation & Benefits

• Competitive base pay: $18-$20/hr

Shift Differentials:

• Evening Differential: +$1.50/hour for hours worked after 7:00 PM ET
• Late-Night Differential: +$2.00/hour for hours worked after 10:00 PM ET
• Weekend Differential: +$1.50/hour for all hours worked Saturday and Sunday

Shift differentials are paid in addition to base hourly wages and reflected in the applicable payroll cycle. Additional Compensation (If Applicable):

• Sign-On Bonus: $2,000 paid after 120 days of continuous employment, contingent upon active employment and satisfactory performance at time of payout.
• Monthly Bonus Potential: Up to $1,000
• 5% Bilingual Pay Allowance
