About the Role: Delivering Clinical Excellence from Afar As a Patient Care Pharmacist, you will be at the forefront of our clinical pharmacy services, providing remote support to our inpatient care teams. This is a unique opportunity to leverage your expertise in a dynamic and innovative environment, working collaboratively with healthcare professionals to ensure the safe and effective use of medications. Our ideal candidate is a self-motivated, critically thinking professional with a passion for delivering patient-centric care. Key Responsibilities: Driving Patient Safety and Outcomes Review and interpret medication orders, ensuring accuracy and compliance with regulatory standards. Monitor patient outcomes, identifying potential issues and collaborating with healthcare teams to optimize therapy. Review patient discharge summaries, providing pharmaceutical care insights to support seamless transitions. Collaborate with multidisciplinary teams to ensure optimal and cost-effective drug therapy, including adjusting dosing and schedules, recommending efficacious therapy, and practicing antibiotic stewardship. Maintain departmental compliance with regulatory readiness, managing records and transactions related to pharmacy operations. Assist in the management of pharmacy inventory, providing information to purchasing teams, removing expired or deteriorated stock, and processing non-formulary requests. Interpret, evaluate, and verify medication orders, demonstrating strong critical thinking and clinical judgment skills. Provide direction and guidance to pharmacy support staff, delegating tasks and ensuring effective workflow. Essential Qualifications: Education, Licensure, and Experience To be considered for this role, you must possess: A Bachelor or Doctor of Pharmacy degree from an accredited institution. A current license to practice as a Registered Pharmacist in the State of Ohio. Effective communication skills, with the ability to collaborate with diverse healthcare teams. The ability to plan, organize, and manage pharmaceutical care, including delegating tasks and providing direction to other team members. The capacity to work independently with minimal supervision, thriving in a fast-paced environment. Preferred qualifications include: A PGY1 residency or three years of institutional pharmacy experience in an advanced clinical setting. A strong background in clinical pharmacy services, with a focus on inpatient care. Skills and Competencies: Driving Success in a Remote Environment To excel in this role, you'll need: Proficiency in electronic medical record systems, with the ability to navigate complex healthcare technology. Strong critical thinking and clinical judgment skills, with a focus on patient safety and outcomes.
