Analyst, Case Management - Field – Must reside in or near Melbourne Florida

They are seeking a Case Management Coordinator who will assess, plan, implement, and coordinate case management activities for members, focusing on enhancing their overall wellness through integrated services.
Responsibilities
- Assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs
- Managing a caseload that includes supportive and medically complex members
- Developing a proactive course of action to address issues presented to enhance short and long-term outcomes
- Determining appropriate services and supports due to members' health needs, including prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, and community resources
Skills
- Must reside in Melbourne FL, or surrounding areas such as Palm Bay and Satellite Beach FL
- Must possess reliable transportation and be willing and able to travel up to 50-75% of the time to meet members face to face in Melbourne and surrounding areas Palm Bay and Satellite Beach. Mileage is reimbursed per our company expense reimbursement policy
- 1 + year of experience in behavioral health or a long-term care setting
- 1+ year of experience in case management and discharge planning experience
- Computer proficiency in Microsoft Word, Excel, and Outlook required
- Bachelor's Degree required, preferably in Social Work or related field
- Bilingual in English & Spanish
- Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
- Effective communication skills, both verbal and written
- Managed care experience
