Job Summary

As a Case Management Analyst at arenaflex, you'll play a critical role in assessing the needs of our clients and developing personalized care plans to address their unique requirements. You'll work closely with our team of healthcare professionals to ensure that our clients receive the best possible care and support. Key Responsibilities

As a Case Management Analyst at arenaflex, your key responsibilities will include:

  • Conducting thorough assessments of clients' needs and developing personalized care plans to address their unique requirements
  • Working closely with our team of healthcare professionals to ensure that clients receive the best possible care and support
  • Identifying high-risk factors and service needs that may impact client outcomes and collaborating with clinical case management or emergency intervention as necessary
  • Facilitating and implementing assigned care plan activities and monitoring care plan progress
  • Collaborating with caseworkers, managers, Clinical Chiefs, and other healthcare projects to overcome barriers to meeting objectives and goals
  • Presenting cases at case conferences to achieve multidisciplinary review and achieve optimal results
  • Recognizing and escalating quality of care issues through established channels
  • Using discussion skills to secure suitable choices and services necessary to meet clients' benefits and healthcare needs
  • Using influencing/inspirational meeting skills to ensure maximum client engagement and promote lifestyle/conduct changes to achieve optimal level of health
  • Providing training, information, and support to empower clients to make ongoing autonomous clinical or potentially healthy lifestyle decisions
  • Assisting clients effectively and knowledgeably to participate with their supplier in healthcare management

Essential Qualifications

* Bachelor's degree in a related field (psychology, social work, marriage and family therapy, counseling, or similar experience)

  • 2+ years of experience in conduct health or social services
  • 2+ years of experience in Microsoft Office applications (Word, Excel, Outlook)

Preferred Qualifications

* Case management and care coordination experience

  • Supervised care experience
  • Education and certification requirements:

+ Four-year degree or non-licensed master's level clinician with either degree being in conduct health or human services (psychology, social work, marriage and family therapy, counseling, or similar experience)
