MTM Pharmacist- Remote

MTM Pharmacist – Telephonic Medication Therapy Management Location: Texas (Remote, must reside in TX, HI, IL, FL, or PA) Schedule: Monday–Friday, 8-hour shift starting between 9:00 AM – 11:00 AM CT Training: Approximately 2 weeks, 8:00 AM – 4:30 PM CT Contract-to-Hire | Pay Rate: $54.05/hr

About the Role

We are seeking a dedicated MTM Pharmacist to join a telephonic medication therapy management team. In this role, you will help improve patient outcomes by identifying and resolving medication-related problems for high-risk members. You will conduct telephonic medication reconciliations, communicate with patients and prescribers, and help members achieve better health through optimized treatment plans. This position requires strong clinical knowledge, excellent communication skills, and the ability to work efficiently in a remote environment.

Key Responsibilities

• Conduct telephonic comprehensive medication reviews (CMRs) , ensuring accurate medication reconciliation and appropriate clinical assessments for targeted members.
• Identify, document, and help resolve medication-related problems.
• Create a clear, health-literacy-appropriate written summary (patient takeaway) after each interaction.
• Prepare prescriber-facing faxed materials to support resolution of clinical issues.
• Build and maintain positive relationships with patients, caregivers, prescribers, nurses, and internal teams.
• Utilize behavioral interviewing and motivational techniques to positively influence patient health outcomes.
• Perform additional duties as assigned by leadership.

Required Qualifications

• Bachelor’s Degree in Pharmacy or PharmD from an accredited school.
• Active pharmacist license in TX, HI, IL, FL, or PA (must reside in one of these states).

Preferred Qualifications

• Previous MTM experience, especially using the Outcomes MTM Connect platform.
• Proficiency with Microsoft Office and strong computer skills.
• Excellent organizational skills, attention to detail, and strong interpersonal communication abilities.
• Experience meeting productivity and quality goals in a remote/work-from-home environment.
• Prior call center experience.
