Medical Assistant (REMOTE)
Location: Fully Remote, Louisiana Schedule: Flexible – Design Your Own Work Hours Position Overview We are seeking a dedicated and detail-oriented Medical Assistant to join our team in supporting remote care management programs initiatives. This fully remote role is ideal for a compassionate professional who excels in patient engagement, chronic disease education, and data-driven care coordination. The successful candidate will Key Responsibilities 1. Patient Communication & Enrollment
- Contact patients to explain the benefits of PharmD Live programs and facilitate enrollment.
- Collect, verify, and submit all necessary information for program enrollment while maintaining a thorough understanding of eligibility and documentation requirements.
- Follow up with patients regularly to track enrollment progress and document updates accurately in the system.
2. Insurance Verification & Prior Authorizations
- Perform insurance eligibility verifications and obtain prior authorizations for services or medications as required.
- Ensure timely and accurate submission of authorizations and maintain records of all communications.
3. Scheduling & Coordination
- Schedule patient appointments in alignment with clinician availability and program priorities.
- Handle inbound patient inquiries, voicemails, and emails promptly and professionally.
- Serve as a liaison between patients, clinicians, and other healthcare providers to ensure smooth coordination of care and continuity of services.
- Contact physician offices or other healthcare facilities as needed to support patient care.
4. Care Coordination
- Facilitate effective communication among patients, caregivers, clinicians, and healthcare partners to support seamless care transitions.
- Track referrals, test results, and follow-up actions to prevent care gaps and ensure patient safety.
5. Documentation & Compliance
- Accurately document all patient interactions, authorizations, and care activities in the Electronic Health Record (EHR) system.
- Ensure full compliance with CMS guidelines for Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs.
- Maintain confidentiality and uphold HIPAA compliance at all times.
6. Program Development & Quality Support
- Assist in the continuous improvement of CCM and RPM programs by providing feedback on workflow efficiency and patient satisfaction.
- Participate in program evaluations and quality assurance initiatives to enhance care delivery.
- Collaborate with cross-functional teams to improve service processes and operational efficiency.
7. Administrative Support
- Manage work queues to ensure timely submission, follow-up, and resolution of all assigned tasks.
- Maintain contact lists, respond to internal and external communications, and act as a point of contact for inquiries.
- Demonstrate strong attention to detail, critical thinking, and follow-through on all assigned responsibilities.
- Perform other duties as assigned to support team operations and patient care goals.
Qualifications
Education & Certification
- Certified Medical Assistant (MA) – Active MA Required
- Additional certifications in chronic disease management, case management, or patient education preferred.
Experience
- Minimum 3 years in working in a doctor’s office performing patient care, care coordination, or chronic disease management duties.
- Experience with AWV, CCM or RPM programs strongly preferred.
Technical Skills
- Proficient in EHR systems and digital documentation.
- Familiarity with RPM platforms and monitoring devices is a plus.
Soft Skills
- Strong communication and interpersonal abilities to support patient education and teamwork.
- Highly organized, self-motivated, and detail-oriented with strong multitasking skills.
Work Environment
- Must have a secure, HIPAA-compliant workspace suitable for telephonic and virtual care interactions.
- Reliable internet connection required for remote work.
