Job Summary
We are seeking an experienced and skilled Full Stack Social Wellbeing Clinical Caseworker (CCM TOC) – Transition of Care Coordinator to join our team at Aetna Careers Remote. As a key member of our team, you will be responsible for providing high-quality care and support to individuals and families in need. Your primary responsibilities will include:
- Conducting comprehensive assessments of clients' needs and developing individualized care plans to address their unique challenges and goals.
- Collaborating with clients, families, and healthcare providers to ensure seamless transitions of care and optimal health outcomes.
- Providing education and support to clients and their families on topics such as medication management, health literacy, and self-care.
- Utilizing advanced clinical judgment and decision-making skills to address complex clinical markers and risk factors that impact care planning and goal achievement.
- Applying evidence-based practices and clinical guidelines to ensure high-quality care and optimal health outcomes.
- Participating in interdisciplinary care planning and coordination to ensure comprehensive and coordinated care.
Key Responsibilities
* Conduct comprehensive assessments of clients' needs and develop individualized care plans to address their unique challenges and goals.
- Collaborate with clients, families, and healthcare providers to ensure seamless transitions of care and optimal health outcomes.
- Provide education and support to clients and their families on topics such as medication management, health literacy, and self-care.

- Apply evidence-based practices and clinical guidelines to ensure high-quality care and optimal health outcomes.
- Participate in interdisciplinary care planning and coordination to ensure comprehensive and coordinated care.
- Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services and support.
- Identify and address quality of care issues through established channels.
- Collaborate with clients and their families to develop and implement care plans that address their unique needs and goals.
- Provide crisis intervention and emergency response services as needed.
- Participate in case conferences and rounds to ensure optimal care and outcomes.
Essential Qualifications
* Bachelor's degree in social work, psychology, or a related field.
- Current, unencumbered licensure as an LPC, LMFT, LFP, or LCSW in the state of Oklahoma.
- 3+ years of direct clinical practice experience post-graduate degree, e.g., hospital setting or elective care setting like wandering care or short-term center/office.
- 2+ years of experience in PCs, keyboard navigation, and MS Office Suite applications (Teams, Outlook, Word, Excel, SharePoint, etc.).
- Strong transportation and willingness to travel in-state up to half of the time. Mileage is reimbursed per our organization's cost reimbursement policy.
Preferred Qualifications

- Experience in case management and release planning.
- Experience in providing care to Native American/Native American/Local American populations.
