Our Home Infusion Therapy Department works closely with physicians, patients, and caregivers to provide intravenous medications and other treatments, ensuring that patients receive the care they need to manage their conditions.

Job Summary

As a Registered Nurse in our Home Infusion Therapy team, you will be responsible for evaluating patients, developing and implementing individualized plans of care, and providing direct patient care. You will work closely with patients, families, and healthcare professionals to ensure that patients receive high-quality care and achieve their health goals. This is a part-time position, with four 8-hour shifts per week, Monday through Friday, and weekend and holiday rotation.

Key Responsibilities

• Evaluate patients and develop individualized plans of care that meet their unique needs and goals
• Provide direct patient care, including assessing and evaluating patients' physical, psychosocial, and emotional needs
• Develop and implement patient education programs to help patients and families manage their conditions and achieve their health goals
• Monitor patients' responses to treatment and modify plans of care as needed
• Maintain accurate and comprehensive patient records
• Collaborate with healthcare professionals to ensure that patients receive high-quality care
• Participate in on-call pharmacy and nursing services, providing 24/7 support to patients

Requirements

Minimum Requirements

• Nursing diploma or Associate's degree in Nursing
• Valid RN license
• Valid driver's license

Preferred Requirements

• Bachelor's degree in Nursing or Home Health
• Less than 2 years of experience in home infusion or a related field
