The Federal government of the United States is seeking an experienced Registered Nurse to join its team as a Telehealth Nurse, providing remote care coordination and patient support to high-risk patients with chronic conditions. As a Telehealth Nurse, you will play a vital role in reducing unscheduled ER visits, unscheduled walk-in clinic visits, and bed days of care by teaching patients the self-management skills needed to stay well.

About the Role

The Home Telehealth nurse is a professional nurse who monitors patient status and coordinates care across the healthcare continuum. Key responsibilities include:

• Coordination of care and disease management education
• Medication management and reinforcement of the clinical treatment plan
• Providing patient support and teaching self-management skills
• Reducing unscheduled ER visits, unscheduled walk-in clinic visits, and bed days of care
• Collaborating with healthcare professionals to ensure comprehensive care

Essential Qualifications

To be eligible for this role, you must meet the following essential qualifications:

• United States Citizenship
• Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies:
  • The Accreditation Commission for Education in Nursing (ACEN)
  • The Commission on Collegiate Nursing Education (CCNE)
• Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia
• Proficient in written and spoken English
• Ability to pass pre-employment physical examination
• Participation in the seasonal influenza vaccination program and COVID-19 vaccination program

Preferred Qualifications

The following qualifications are preferred but not required:

• Two or more years of Case Management or Care Coordination experience
• Prior Home Telehealth experience
• Two or more years of Medical-Surgical experience
• Bachelor's degree in Nursing or related field
