Key Responsibilities

• Deliver telephonic or virtual clinical support to patients

• Coordinate care plans with physicians, specialists, and other providers

• Monitor patient health data and respond to alerts or symptoms

• Educate patients on chronic condition management

• Maintain accurate electronic health records in compliance with HIPAA

Qualifications

• Active RN license (must be valid in at least one U.S. state)

• Minimum 2 years of nursing experience

• Previous experience in case management, telehealth, or chronic care preferred

• Strong communication, assessment, and documentation skills
